About Diagrama

Diagrama Foundation is a charity based in the South East that supports vulnerable adults and children. We believe in assisting the development of core skills for adults with learning disabilities and improving quality of life by supporting independence. We focus on developing skills and confidence that lead to more independent, fulfilled lives, community integration and wellness.

Meet Us

Meet Keri

Keri is our Care Farm and Community Manager. Her experience, with her skilled team,  will help to make sure you not only have a wonderful time, but will gain valuable skills for life and work.

Meet Jason

Jason is our Care Farm and Community Coordinator. He will support Keri and the team of Instructors to deliver a great quality service.

What We Do

Diagrama provides a choice of meaningful, enjoyable occupational opportunities and activities in different locations, so attendees develop skills that could transfer to an employment opportunity and daily living skills, promoting independence.


 

Gardening and Food Crop Production
Growing vegetables, fruit & flowers and looking after nature.
Wood Working
Creating bird boxes, planters, hedgehog houses & fixing fences.
Woodland Crafts
Traditional skills in carving, weaving, and rustic furniture.
Cooking and Catering
Menu planning, healthy eating and cooking simple meals.
Animal Care
Looking after and caring for a range of animals across the farm.
Land Management
Sustainable farming and biodiversity cultivation.
Art & Craft
Pottery, blacksmithing, and traditional textiles.
Daily Life Skills
Sessions on online safety, managing money and more.

Locations

We deliver these activities in three different locations.

Duckyls Farm

Duckyls Farm, near East Grinstead, boasts beautiful open spaces and is home to both woodland and farm animals. 

The Cherry Tree Centre

A community centre in Burgess Hill where our cooking, catering, art & craft sessions and life skills workshops are held.

The Yews

A multi-purpose community centre in central Haywards Heath where we deliver art & craft and life skills workshops.

Information For Family Of Service Users

Diagrama has been awarded the contract to provide Day Opportunities, Hub and Outreach Support for people with a learning disability, physical and sensory impairment, acquired brain injury and/or autism. Diagrama will work with Signposts to deliver these services. This means that your loved one will be offered to attend our Day opportunities services. We are looking forward to meeting you all.

What’s important to know about our support

We are passionate about providing the best possible support for the people we have the privilege to work with. We aim to improve lives through a person-centred approach. Our service model supports people to do as much as possible for themselves and live meaningful, enjoyable lives. We build on service users’ strengths and capabilities and have worked with other organisations to refine our ways of supporting people, including with Montessori experts in disability and dementia care.

We believe everyone deserves to have a voice in the service – including family members. We welcome your insight and are looking forward to working together with you as one united team.

The management of the West Sussex Day Opportunities, Hub and Outreach Support was awarded to us on 20 May 2024.

"This is an exciting opportunity for us as we are deeply passionate about providing excellent care. Our day services in West Sussex open up a new set of choices for adults with learning disabilities in the county and we know that our 100 acre farm will be a fantastic skills enhancing environment for anyone to spend time in, alongside the valuable experiences offered at the community centres. For this contract, we will work in partnership with Signposts who will continue to provide the Hub and Outreach services in Mid Sussex. We look forward to welcoming new Service Users and them being involved in the future development of our farm and day opportunities services."

Our Partners

In Mid Sussex, we work in partnership with Signposts. Signposts provide the Hub and Outreach Support service, with local drop-in services in Burgess Hill, East Grinstead and Haywards Heath during the week, offering information, advice, flexible support, activities, learning and social opportunities.
